Excruciating Pain from Endometriosis: The Seed Cultivated by a Cold Womb

The concepts of a cold womb, blood stasis, and qi stagnation described in the Donguibogam align closely with the mechanisms of endometriosis in modern medicine. Endometriosis occurs when endometrial tissue, which normally lines the inside of the uterus, implants and grows outside it, such as on the ovaries, fallopian tubes, or peritoneum. This ectopic endometrial tissue thickens and bleeds with each menstrual cycle, but the blood cannot exit the body and remains trapped within the abdominal cavity. This trapped blood triggers an inflammatory response and irritates surrounding tissues, producing a chronic inflammatory state. This parallels the concept of blood stasis accumulating internally. The ongoing inflammation irritates local nerve endings, causing not only severe dysmenorrhea during periods, but also chronic pelvic pain between cycles. When central sensitization develops because of persistent inflammation, the nervous system becomes hyperreactive, lowering the pain threshold so that even mild stimulation causes intense pain. This reflects how qi stagnation worsens physical discomfort. Endometriotic implants can also cause tissue adhesion, binding pelvic organs together. These adhesions pull on organs whenever the body moves, causing ongoing pain and frequently leading to dyspareunia. When an endometrioma develops on the ovary, commonly referred to as a chocolate cyst, it can cause dysmenorrhea and contribute to infertility. Modern medicine identifies several contributing factors to endometriosis, including genetic tendencies, immune dysfunction, hormonal imbalances like estrogen dependence, and retrograde menstruation. In this context, the traditional concept of a cold womb serves as a meaningful framework for understanding impaired pelvic circulation, chronic inflammation, and functional decline in the endocrine and immune systems. When pelvic blood flow is poor and inflammation persists, the body creates an environment vulnerable to worsening endometriosis, much like cold ground harboring troubled growth. The principles in the Donguibogam offer a clear way to understand and care for these systemic imbalances in everyday life.
