Back pain caused by weakened bones should not be dismissed simply as an inevitable part of aging. It could be the first sign of an osteoporotic spinal compression fracture. Osteoporosis is a condition where bone strength declines, significantly raising the risk of fracture. It occurs commonly in postmenopausal women and older men, developing when the activity of bone-destroying cells (osteoclasts) outpaces that of bone-forming cells (osteoblasts), leaving the interior of the bone porous and hollow, much like a honeycomb sponge.
These weakened bones, especially the vertebra, can break easily even under minor stress. Beyond falls or lifting heavy objects, a compression fracture where the vertebra crumbles and collapses can happen from a cough, a sneeze, or simply turning the body. Initially, it may feel like a minor sprain that is easy to overlook, but the pain steadily intensifies over time, accompanied by a stooped back and loss of height. In severe instances, it can cause nerve compression, leading to numbness or weakness in the legs.
Diagnosing an osteoporotic compression fracture requires bone densitometry, x-ray, and magnetic resonance imaging. Magnetic resonance imaging is especially critical for identifying the timing of the fracture and confirming whether there is nerve compression. Treatment begins with pain management, followed by pharmacotherapy to rebuild bone strength, such as bisphosphonates, parathyroid hormone preparation, or denosumab. When necessary, procedures that inject bone cement into the collapsed vertebra, such as vertebroplasty or balloon kyphoplasty, may be considered. Prevention remains paramount. Sustained efforts to safeguard bone health should include adequate intake of calcium and vitamin D, regular weight-bearing exercise, and home modifications to prevent a fall. Just as Donguibogam emphasized the vital role of the kidney, modern medicine stresses whole-body wellness and cultivating the fundamental strength of your bones.
