Tingling Thumbs: Self-Diagnosis for Texting Wrist Syndrome Caused by Smartphone Addiction

For anyone relying daily on a smartphone, thumb pain is a familiar complaint. While the Donguibogam speaks of the consumption of qi and blood and stagnation, modern medicine identifies this condition as de Quervain tenosynovitis. Several tendons are involved in moving the thumb. On the thumb side of the wrist, two key tendons run together: the abductor pollicis longus (APL) and the extensor pollicis brevis (EPB). These tendons are encased in a protective sheath known as the tenosynovium, structured much like a narrow tunnel. The sheath contains synovial fluid that acts as a lubricant, enabling the tendons to slide smoothly. Problems begin when we grip a smartphone and overuse the thumb, or perform repetitive movements with the wrist bent at unnatural angles. Constantly repeating abduction (moving the thumb outward) and extension (straightening the thumb) causes continuous friction between the APL and EPB tendons inside that narrow tunnel. Over time, persistent friction leads to inflammation and swelling of the sheath. As the sheath swells, the tunnel becomes even tighter, restricting tendon movement further and creating a cycle of escalating pain. This pain is concentrated from the base of the thumb down to the wrist, often presenting as a sharp or burning sensation when gripping objects or moving the thumb. In more severe cases, numbness or paresthesia may extend into the wrist. A common self-assessment for de Quervain tenosynovitis is Finkelstein's test: fold your thumb into your palm, wrap your fingers around it to make a fist, and bend your wrist sideways toward your little finger. Experiencing sharp pain at the base of the thumb suggests a strong likelihood of de Quervain tenosynovitis. A definitive diagnosis should always be confirmed by a medical professional.
