Dear patients, the sudden turmoil disorder described in the Donguibogam closely parallels today's acute gastroenteritis, severe food poisoning, and particularly Vibrio sepsis. Vibrio sepsis is a serious infection contracted by eating raw seafood contaminated with Vibrio vulnificus bacteria or by exposing open wounds to tainted seawater. Because these bacteria proliferate rapidly in warm seawater, infections concentrate heavily between May and October, especially through midsummer.
The illness initially presents like standard food poisoning, with fever, chills, vomiting, diarrhea, and abdominal pain. The primary danger, however, is that it can advance rapidly to sepsis. Sepsis occurs when the body mounts an uncontrolled response to bacterial infection, triggering systemic inflammation. What the Donguibogam described as blocked flow of qi and turbid toxins overtaking the body corresponds clinically to systemic inflammatory response syndrome. When inflammation becomes overwhelming, blood vessels dilate excessively and blood pressure plummets, causing septic shock. Inadequate blood delivery to essential organs—including the liver, kidneys, lungs, and heart—induces multiple organ failure, an outcome that can prove fatal. Among patients with underlying liver disease, chronic disease, or compromised immunity, the fatality rate exceeds 50 percent.
Because Vibrio vulnificus carries potent toxicity, characteristic skin lesions often emerge within 24 to 48 hours of infection. These begin as red patches, advance into blisters, and can culminate in tissue necrosis. Such clinical findings align with the ancient description of pain that twists the body near death. Although historical physicians lacked microbiological concepts, they correctly appreciated the severity and rapid course of this illness through careful observation. During the summer months, eating fresh, properly prepared food and maintaining hygiene provide protection no less essential than any vaccine.
